Our service includes the use of physical exercise and/or medications. For your safety, we need to ensure you are suitable to perform exercise and/or use any medications we may provide you with. The health form is all about you, your history and your goals. Your information will NEVER be shared with any 3rd party except our pharmacy partners, who dispense your medication. Please select your treatment option below to complete the relevant consultation - treatment is subject to approval. Please note, we only ship to UK residences.

Please select below if you are a NEW customer (new to us as a customer, even if you have used the medication before from elsewhere, OR if you have not been with us for 12 months as we will require an up-to-date health form) - We will require a form of ID to verify your identity. If you have used the medication before, we will require proof of previous usage.

Please select RETURNING customer if you have previously received treatment from us within the last 12 months.
