Why do I need to complete a health form?

As this is a prescription-only medication, we need to ensure you are suitable to use this medication. Your information will NEVER be shared with any 3rd party. We only share your information with our prescribers pharmacy suppliers, who dispense your medication.​

Please note, this medication is for those 18 years old to 65 years old. If you are over 65, you would need to be assessed in person by a specialised clinic or your GP. As an online telemedicine provider, we cannot provide it for you if you are outside this age range.

What happens after I have done the form?

Your form will be screened by a prescriber and we will contact you with the outcome as soon as we hear. This can take up to 48 hours to process.
